Payment
Cash, Visa, Mastercard and Discover
are accepted. Full payment is expected at the time of services unless
previous arrangements have been made. Gratuities are not accepted
for professional services and against the nursing code of ethics.
Cancellation
We
all value our time. Notice for cancellation within 12 hours
of your appointment is required; 24 hours is preferred. Please
call as soon as possible if you need to cancel or reschedule. Missed
appointments without cancellation except in the case of emergency
will result in you being charged for half the amount of the scheduled
service.
Privacy
Your
person privacy and medical information will be kept in strict
confidence. Information obtained from you regarding health, medications,
treatments, medical history and financial information will not be
released to any institution, organization, medical personel or
other perons without your written consent or request.
